Evaluation Forms

Please find the 2010 6 month and final evaluation forms posted here. Should you have any questions about completing the forms or would like them in Microsoft Word format, please contact jdoyle@tfec.org. If your project is 100% complete by the 6 month mark, please utilize and submit the Final Evaluation Form and your file will be considered complete and closed. Please submit these documents by the due dates listed in your grant agreement and award letter.
